Insurance claims are a particularly rich target for deepfake fraud because the claim evidence is the media. An image is the proof of damage; a video is the proof of injury; a voice recording is the proof of a verbal agreement. When any of those can be generated or manipulated at low cost, the adjustment process itself becomes the attack surface.

Where deepfakes show up in claims

Auto physical damage. Staged-incident photos generated to match the geography and vehicle of a real policy. A diffusion model can produce realistic collision imagery that matches a specific car model and location from a text prompt. Pair with a fake police report PDF and the claim packet looks complete.

Property and catastrophe. In the wake of a disaster (hurricane, wildfire), claim volume spikes and adjuster capacity is thin. Attackers submit AI-generated photos of damage to uninsured or uninvolved properties, banking on volume to push claims through without physical inspection.

Health and medical. Synthetic injury videos, AI-generated medical imaging, and fabricated treatment documentation. Harder to detect because the ground-truth "what did the injury actually look like" isn't independently verifiable.

Life and disability. Cloned-voice recordings submitted as evidence of an oral promise, a deathbed change in beneficiary, or a disability-onset declaration. Rarely the primary evidence in a contest, often a secondary pressure point.

Detection workflow that fits an adjustment pipeline

Claim adjustment is a latency- and cost-sensitive process. Deepfake detection has to fit inside the existing workflow, not replace it:

  1. Intake scan on every submitted media asset. Photos, videos, voice memos. Automated image, video, and audio detection runs in parallel.
  2. Fraud-model integration. Detection scores become another input to the existing fraud-scoring model, weighted alongside claim-history signals, location risk, and adjuster-assigned risk flags.
  3. Threshold-driven escalation. High detection-confidence flags go to the SIU (Special Investigation Unit); mid-range flags get human adjuster review; low scores pass.
  4. Evidence retention. Detection output — score, model version, timestamp — persists alongside the claim file for regulatory, reinsurance, and potential civil litigation purposes.

Governance and documentation

Insurance is a heavily regulated sector. Three patterns the best-adopting carriers follow:

  • Vendor documentation. Model accuracy on real claim data (not just public benchmarks), update cadence, and false-positive rates by claim type — all required for regulatory examination.
  • Audit trail by default. Every detection decision logged with inputs, outputs, and the adjuster action that followed. Required for post-hoc review and disputes.
  • Customer-facing disclosure. Policy language permitting AI-assisted review of submitted evidence; most US states now require this for meaningful adoption.

Where the market is heading

Two trends to watch through 2027:

  • Reinsurance pressure. Reinsurers are starting to ask carriers about their deepfake-detection posture during treaty renewal. Expect this to accelerate primary-carrier adoption.
  • Regulatory clarity. NAIC's model bulletin on AI in insurance underwriting and claims, combined with state-level action, is converging toward a framework that effectively mandates detection on high-value claims.
